Direct billing is offered with some providers. If that is the option you choose, prior to your appointment, you must ensure that your plan covers massage therapy and whether a doctor's note is required by your Insurer.
Please bring me your plan membership card, ID and I will take care of the rest.
Any portion not covered by insurance is due at the time of service.
If your claim is not accepted by the Insurer or if I'm unable to process your claim due to technical reasons beyond my control, you must pay in full.
