Do not forget to complete the Intake form and the Insurance Policy Form (if you need direct billing) before your first appointment.
I'm located at 46 Hartwick Manor, Spruce Grove.
Please arrive 5 minutes before your scheduled appointment to ensure that you receive your full treatment.
